The Vial of Life program is designed to provide Emergency Service Personnel with time saving medical information in the home and automobile. For more information or to receive additional kits, please contact the Melbourne Village Police Department.

The kit includes a vial, 2 stickers, a rubber band, and 2 lists where 2 people can describe
their medical history. The lists go inside the vial. It good place to put the vial is in
the refrigerator. This way, if there is a medical emergency, you can tell the emergency
workers about it when they get to your house. Then they know what your medical history is.
